In addition, our hospice physicians are specialists in hospice and palliative care medicine and understand the treatment modalities for advanced diseases in order to achieve comfort and symptom control.

Community Hospice of Victor Valley Referral Center

Our office center staff will answer questions, arrange for services, and clarify insurance coverage. A nurse may be assigned to work directly with a patient’s hospital, rehabilitation facility, nursing home, case manager, or physician to ensure a smooth transition home.

Community Hospice of Victor Valley (CHVV) is a non-profit, community based agency providing hospice care for people of all ages when they can no longer benefit from health healing treatment. Hospice is a program to help you care for someone with a life limiting illness. We provide a supportive service that helps provide comfort care to patients and emotional support for family members.
